PHASE MINUTES OF MEETING No. 001/2008

ATTENDANCE:

Members + officers + honorary = 48

New comers for membership = 18

AGENDA;

Ø The meeting was presiding by Mr. Antonio Dilao.

Ø Mr. A. Dilao announce the PHASE SAFETY THEME FY 2008 (attach) and also there will be safety card tips (to distributed) every meeting.

Ø Mr. A. Dilao stated that the upcoming 5^th induction of new member including new sets of OFFICERS FY 2008 will be on February 29, 2008 (Friday) to be held at MOVENPICK Tower & Suites Doha, West Bay Area. Time from 4:00pm to 10:00pm, cost of dinner per member is QR250.00.

(ALL NEW MEMBERS ARE REQUESTED TO PAY THE DINNER COST ON JANUARY 18, 2008, at least 50% due we have to pay the hotel in advance prior to confirmation of the event and the another 50% need to be fully pay one week before the event.) Please confirm your intention to attend for we need confirm attendees due the venue capacity is maximum of 140 persons only. Those who wish to confirm his intention may contact your PHASE secretary by phone of email for booking.

Ø Mr. A. Dilao announces the Scheduled Seminar for PHASE members will be on January 25, 2008 (Friday) will held at Guest Palace Hotel, Doha, Qatar. Time is from 8:00am to 5:00pm. Topics are P.P.E – SCAFFOLDING / LADDER – EXCAVATION – FALL PROTECTION. Fee for attending the seminar is QR150.00 (including lunch & snacks). To all members interested should book their intention to PHASE secretary and requested to pay the said amount on the next meeting.

Ø Safety THEME for January 2008 was conducted by our Ways & Means Chairman Mr. Fernando N. Almonte (Job Hazard Analysis & Risk Assessment) & Mr. Francis E. Tan, PHASE Vice President (Role of Safety Practitioner).

NOTE: The same topic will be presented on the next meeting (Jan. 18, 2008).

Ø The next meeting will be on January 18, 2008 (Friday) at Philippine School Doha (PSD), from 1:00pm to 4:00pm.
